Physical Therapist

Major Responsibilities

Responsible for providing physical therapy services in order to achieve optimum treatment, habilitation or rehabilitation for individuals whose ability to function is impaired by congenital or developmental disability, physical illness or injury.

Specific Duties

  1. Evaluation of patients for service needs
  2. Perform physical therapy procedures as ordered by referring physician
  3. Participate in the formulation of the patient’s plan of treatment and revise as necessary
  4. Participate in program planning and team conferences
  5. Assist in developing physical therapy policies and procedures
  6. Confer with Director of Early Childhood Classroom Program concerning patient’s care and activities as needed
  7. Participate in the delivery of in-service education programs
  8. Record on patient charts all appropriate physical evaluations, progress notes, consultations and discharge summary information
  9. Submit all notes, evaluations and required forms within agency guidelines
  10. Provide in-home education to the family of the patient’s physical therapy needs
  11. Maintain such records and prepare such reports as requested by the agency
  12. Participate in formal staff training and formalized in-service training as appropriate
  13. Make recommendations for purchases of specialized program materials, supplies and adaptive equipment
  14. Collaborate with other team members or those individuals designated by the agency (e.g. family members, teachers, speech-language pathologists, consumers, etc.) to facilitate comprehensive treatment
  15. Assists in training physical therapy students

Knowledge, Skills and Special Requirements

  1. Thorough knowledge of the principles and practices of physical therapy
  2. Thorough knowledge of standards of clinical practice established by the American Physical Therapy Association
  3. Ability to exhibit sound, professional judgment

Experience

New York State Licensed and Registered as a Physical Therapist Graduate of a Physical Therapy curriculum accredited jointly by the Council of Medical Education of the American Medical Association and the American Physical Therapy Association
